Speciality Doctor/GP in Perth & Kinross

Job description

Applications are invited for speciality doctors/GP with special interest to join Perth and Kinross (P&K) HSCP to work within and help shape a Hospital @ Home Service, with support from the Medicine for the Elderly team.

We are looking for an enthusiastic individual with wide ranging medical or GP experience with a keen interest in caring for Older People who would want to work to develop and support our new Hospital at Home (H@H) service in P&K. The H@H model aims to act as an important connection between acute care provided at Perth Royal Infirmary (PRI) and community case provided by GPs and associated community teams. The Medicine for the Elderly team will support Hospital @ Home and the post-holder could also develop roles in Frailty at the Front Door and Intermediate Care. The individual would be a team player and will contribute to service development of H@H improvement as part of their post.

Perth and Kinross is undergoing significant change within our service models and we are developing community models of care to support strategic direction of our Medicine for the Elderly services covering Hospital at Home, Frailty at the Front Door and Intermediate Care.

The Medicine for the Elderly service works across three localities in Perth and Kinross, community hospitals and secondary care and is based at Perth Royal Infirmary. It is undergoing a period of expansion, change and innovation. At present the Medicine for the Elderly department consists of 5 consultants, 3 speciality doctors and several clinical fellows and 3 ANPs and provides a comprehensive service to older patients in Perth and Kinross.

At Perth Royal all frail older people are assessed by the acute frailty team on admission and then either discharged early or cared for in one of our MFE clinical areas: the acute MFE in-patient unit, the orthogeriatric and short stay rehab unit or the slow stream rehab unit. Pathways of care into the community function via the two community hospitals that are under the care of MFE and are undergoing active service development.  The out-patient care of the older population is delivered through a rapid assessment and fall service, Parkinsons service and evolving services based in the community with the ambition to achieve seamless working with the community teams.

Perth & Kinross Health & Social Care Partnership is in the early stages of implementing a Hospital at Home Model in one locality of Perth & Kinross, specifically Perth City which is an urban area. We have recently recruited to the majority of the clinical workforce, who are now going through induction whilst also being involved in the development of the model.

​​​​​​​The dedicated Hospital at Home Team currently comprises of:

  • Service Manager
  • Nurse Consultant Urgent Care
  • General Practitioner with Specialist Interest
  • Occupational Therapist
  • Physiotherapist
  • 3.45 Nurse Practitioners
  • 3.45 Healthcare Support Workers
  • Project and Administrative Support

These posts would primarily support development and clinical oversight of patients on the Hospital at Home/Urgent Care service, but more broadly support the developing models of care across Frailty at the Front Door and Intermediate Care. Broader service development experience and clinical experience within medicine of the elderly settings will be encouraged and supported, with the post acting as an opportunity for significant clinical development.

The successful candidate can work fulltime or part time.

There would be opportunity to discuss working in a dual role of general practitioner for the HSCP along with an MfE or potentially other speciality component depending upon candidate experience and interests.
