Community Matron - End of Life and Supportive Care - COVID-19 in Oxford

The post holder will be part of the recently formed countywide Integrated Intensive Community Care Pathway leadership team which has a strong emphasis on admission avoidance to hospital . The Community Matron role will have a major focus on frailty and those patients who have complex and escalating needs. You will work with patients and their families to ensure a personalised approach to care planning.

In the Integrated Intensive Community Care Pathway, you will be working closely with other professionals such as nurses, doctors, physiotherapists, occupational therapists and a variety of support staff. You will also work alongside other countywide district nursing, specialist nursing and therapy services who offer Planned and Preventative care interventions.

Strong interdisciplinary team working skills are essential to ensure that colleagues from the integrated teams are all focused on the needs of the patient, working in a joined up way to help patients to achieve their goals. Some hands on clinical care will be required.

Good co-ordination, prioritisation and management skills will all be required in this interesting and dynamic role. The sharing of knowledge and clinical expertise with colleagues will be key. The post will also have a strong focus on education and teaching.

The post holder will need to be both compassionate and resilient as this is potentially an emotionally draining role. This is where really good team working is key , with the shared responsibility of meeting patient need.

Strong and confident communication skills are needed in the Matron role , both on an individual and group level. The Matron will liaise regularly with key stakeholders such as Primary Care colleagues, Acute Hospitals, Hospices and care providers. The Matron will also be part of key working groups within Oxfordshire as we further develop the frailty and end of life care pathways.

As one of the 7 national accelerator sites for NHSE Ageing Well agenda, you will be joining the service at an exciting time as Oxfordshire develops the Two Hour and Two day Community Response . This role offers the opportunity to help shape community service provision for the future.

The base for this post is on the outskirts of the vibrant city of Oxford.

All interviews will be held virtually via Microsoft Teams.

Please note: The service currently operates over 7 days a week, so flexible working into the evenings, over some weekends and bank holidays will be required. The post is available full-time (37.5 hours per week).
