Supported Discharge Team, (Stroke) Aberdeen City CHP

Hours – 18.75 per week

Details:  An opportunity has arisen for a qualified Occupational Therapist to join the Supported Discharge Team, based in Aberdeen. The supported discharge team provides stroke specific rehabilitation in the community and enables individuals to complete their rehabilitation at home rather than in the hospital. The post holder will work as part of a multi-disciplinary team providing a client centred/goal orientated approach to invention and treatment. The team are very supportive, with a shared approach to inter-professional learning and sharing of good practice.

The supported discharge team is a time limited service providing up to 6 weeks post discharge intervention to patients and carers, with the aim of assisting patients to self manage their condition.

The post holder will manage a complex adult stroke caseload requiring highly specialist assessment, diagnosis and treatment and support a Band 6 and Band 4 HCSW.

There is currently a review of the Neuro Rehabilitation Pathway within the City and this team will be part of that review.
