Working 37.5 hours per week when you join the team at Pinhoe View in Exeter and embark on a career where you are valued and supported in everything you do as a Discharge coordinator and family / carer link.
As a Discharge coordinator and family / carer link you will support both the nursing team and wider MDT with both simple and complex discharges. They will also provide the required link between the wider MDT, nursing team and family, friends, and carers
You will also work closely with other agencies, family and carers and the ward teams allowing them to be proactive in identifying potential delays and have the skills to generate and pursue solutions. This will not only enhance patient experience by ensuring a safe and efficient discharge from hospital but also support family, friends and carers to also have a voice in their loved one’s care.
Your responsibilities will include:
- To monitor themes and trends, arising from feedback being conducted with service users, also family and carers. To feed those trends back to the wider teams
- Support the discharge process by providing the background information on referred patients within the unit by liaising with external teams
- To work autonomously within their scope in, providing comprehensive advice and information to ward staff, patients, and relatives, operationally and around discharge.
- To have a knowledge and understanding of Power of Attorney and Court of Protection and how these impacts on patient plans including Power of Receivership; Mental Capacity Act; Deprivation of Liberty and other legal frameworks.
- Support the identification of patients who would fit into the parameters of Continuing Health Care and whose needs are high intensity health needs and support referrals
- To take the lead for family carer forums/events for the hospital promoting engagement from families and carers.
- Work with the ward team where required to provide a ward-based response to supporting both simple and complex discharges in a defined group of patients
- Act in a way that safeguards the well-being and interests of all service users, employees, and visitors.
To be successful in this role, you will have:
- Discharge planning in a hospital setting (desirable)
- Awareness of the Care Act 2014
- IT Skills and use of computer packages
- Ability to communicate in a variety of settings
- Able to manage own workload
- Manage time effectively
- Health and safety level 3 or above qualification (desirable)
- Valid UK Driving Licence
