Are you a qualified Adults Social Worker seeking a role that makes a real difference? Pertemps are working with a supportive and forward-thinking Hospital Discharge Team in Merseyside to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals to ensure a smooth transition for patients from hospital to home or other appropriate settings.


Adults Social Worker Role Overview

As part of this service, you will play a crucial role in assessing the needs of adults, facilitating discharge planning and providing ongoing support to individuals and their families. Your help will improve and reduce hospital stay and prevent readmissions.


Adults Social Worker Key Responsibilities

  • Conducting comprehensive assessments of patients needs and circumstances.
  • Developing and implementing person-centred discharge plans in collaboration with multidisciplinary teams.
  • Providing emotional support and guidance to patients and their families during the discharge process.
  • Ensuring compliance with relevant legislation and best practice guidelines.
  • Maintaining accurate case records and documentation in accordance with organizational policies.


Adults Social Worker Requirements

  • A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • At least 3 years post-qualifying experience
  • Current registration with Social Work England
  • Experience in adults social care, hospital discharge or D2A services is essential
  • Strong communication and interpersonal skills, with the ability to work effectively in a team
  • Knowledge of relevant legislation, policies, and procedures related to adult social care
