Pertemps Social Care are recruiting agency social workers for roles within a Hospital Team based in Huddersfield.

About the Role

You will be working as part of a multi-disciplinary team, assessing and facilitating safe and timely discharges with a commitment to promoting independence.

Responsibilities

• Rapid Assessment under Discharge to Assess guidelines for hospital discharge
• Assessing service users, mental capacity and making best interest decisions
• Liaising with the discharge liaison team, as well as ward staff and family members.
• Arranging care packages on discharge and step down beds.
• Completing reviews and support plans.
• Reporting any delays with service users.
