Job description
OverviewYou will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.
Responsibilities
Deliver quality care to a designated caseload in the home setting.
Follow a care plan devised by the community nursing staff you will be working alongside.
Provide concise daily handover feedback to ensure changing needs are met and risks escalated.
Be flexible and assist colleagues to cover service needs across the wider integrated team.
Support service delivery during operating hours 08:00–21:00 with rotational shifts including weekends, evenings and Bank Holidays.

Qualifications
Dynamic, motivated and enthusiastic with excellent communication skills
IT literate and good problem solving abilities
Passion for patient-centered care
Ability to work across the wider integrated team and flexibility to cover service needs
Ability to provide care to adults over 18 living in Bedfordshire
