37.5 per week. Consisting of 4 x 9.5 hour split shifts, worked on a rota basis between 08:00 and 22:00 including Bank holidays and weekends.

About this jobWe are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver a range of quality care services to patients visited by the Community Response Hub based at Tarporley Hospital.

The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.

Applicants must demonstrate how they will meet the travel requirement for this post.

Applicants require end of life care experience to be considered for this post.

This post involves lone working and requires travel across the Western Cheshire footprint. Access to transport for work purposes is essential and must be demonstrated in your application.

Key responsibilities

Experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and experience of providing care for palliative patients.

You are encouraged to enrol for the DBS Update Service. Please note applicants will be required to pay for their DBS check and costs are deducted from salary following appointment.
