About the Company

North Bay Group are looking for Care Plan Managers to drive excellence, consistency, and quality in care planning across our homes.


We have 3 vacancies available and ideally you will live in either Cheshire / North West, West or North Yorkshire of South Yorkshire.


The Care Plan Manager will oversee quality, accuracy, and consistency of care planning across North Bay Group’s care homes. The role ensures that care plans are person-centred, clinically robust, compliant with regulatory and contractual requirements, and reflective of residents’ assessed needs and wishes.


Working in partnership with Home Managers, clinical teams, and quality colleagues, the Care Plan Manager provides oversight, guidance, and assurance rather than direct line management, supporting services to maintain high standards of documentation and care delivery.

Responsibilities

  • Elevating the quality and consistency of care plans across our homes
  • Ensuring care is person-centred, accurate, and outcome-driven
  • Supporting teams through coaching, training, and practical guidance
  • Leading audits and driving continuous improvement
  • Ensuring compliance with regulatory and best practice standards
  • Working collaboratively with Home Managers and clinical teams


Qualifications

  • Strong experience in care planning within health or social care

  • Required Skills

    • Experience of PCS and/or Nourish is required
    • A keen eye for detail and a passion for quality
    • Confidence in auditing, improving, and influencing practice
    • Excellent communication and coaching skills
    • A commitment to high standards and resident wellbeing
