Advanced Nurse Practitioner/ANP Trainee

Job description

This is an interesting and exciting opportunity within the multi‐professional dementia team. The role includes facilitating rapid access to a diagnosis, treatment, crisis prevention and intervention, preventing avoidable hospital admissions and assisting with timely discharge from hospital.

The Service comprises three teams staffed with mental health nurses, occupational therapists, social workers, senior health care support workers and Alzheimer Scotland post‐diagnostic link workers. The teams are part of the Argyll and Bute Enhanced Community Dementia service and work jointly with Community Health Services, Mental Health, Primary Care and other agencies including social care.

Responsibilities

Provide clinical assessment and care planning in partnership with service users and carers.

Facilitate rapid access to dementia diagnosis and treatment.

Lead crisis prevention and intervention activities.

Support the prevention of avoidable hospital admissions and timely discharge.

Collaborate with multidisciplinary teams across community and health services.

Qualifications and Experience

Experience working with people living with dementia.

Master's qualification in Advanced Practice or ability to undertake training as a trainee ANP under Annex 21.

Open to qualified advanced nurse practitioners, existing band 7s and experienced band 6 nurses willing to undertake training to become a Band 7 ANP.

Willingness to undertake study at master's degree level if trainee.

Appointment may be on a training basis under Annex 21, paid according to NHS terms and conditions.
