Health Care Assistant – Urgent Community Response TeamClosing date: 19 May 2026
Job OverviewSupport people in the local community to regain independence following accident or illness. Work in patients' homes, assisting with activities of daily living, personal care, meal preparation, and social care. Collaborate with therapists and social workers to help patients achieve recovery goals.
Key Responsibilities
Implement assessed care needs and maintain progress in rehabilitation / reablement.
Assist patients with personal care tasks delegated as part of the reablement care plan, including meal and drink preparation if required.
Provide and assist with care in patients' homes, supporting a person‐centred approach and recording observations, applying topical cream.
Support admission prevention by responding to incidents of carer breakdown and providing rapid support to prevent hospital admission.
Assist with medication management as per care plans and policies.
Required Qualifications
NVQ Level 3 in Health and Social Care or demonstrable equivalent experience.
Valid driving licence and suitable vehicle for travel between patients' homes.
Personal Qualities
Willingness to undertake in‐service training and development.
Ability to work shift patterns over a 24‐hour period (7‐day rota, 8 am – 10 pm).
Access to a car for work purposes.
Caring attitude and strong desire to help people.
Good communication and interpersonal skills.
Adaptable, reliable and approachable.
Skills & Attributes
Good organisational skills.
Ability to work unsupervised when required.
Flexibility in role.
Basic computer skills.
Experience & Knowledge
Experience of patient care in the community.
Experience of working within a team and under pressure.
Knowledge of promoting independence, infection control and tissue viability.
Qualifications & Training
Knowledge of infection control.
Knowledge of tissue viability.
Care Certificate (or equivalent).
Disclosure and Barring Service CheckThis post requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
