Eligibility Requirements• Must have access to a personal vehicle.
• Must have the right to work in the UK; we are not able to provide sponsorship.
• Hourly rate is subject to holding an NVQ level 2 or above in Health and Social Care.
Responsibilities
Help people leave hospital as soon as practicably possible and support their safe transition back to independent living.
Visit Service Users in their own homes to provide care and support as defined in their Care Plan.
Perform all aspects of personal care, administer medication, monitor vital signs, encourage exercise, and assist with domestic tasks as needed.
Liaise with medical professionals such as district nurses and pharmacies.
The aim is to rehabilitate and maximise potential, supporting Service Users in a range of activities to promote independent living and monitor their well‐being.
Working HoursShifts run between 7 am and 10 pm, with patterns of 7 am–2 pm and 4 pm–10 pm, including alternate weekends.
Pay RatesHigh pay rates of £13.43 ph on weekdays and £15.50 ph on weekends.
