Healthcare Support Worker – Older Persons Mental Health CommunityThe closing date is 25 May 2026

Exciting opportunities across our Older Persons Mental Health Community Team in Basingstoke, Winchester, Fareham and Gosport.

As a Healthcare Support Worker, you will provide diagnostic support to patients with dementia and other mental health diagnoses, help maintain their wellbeing and independence at home, and support families and carers. For the Basingstoke role patients will be visited in a care home setting.

Compassionate and high‐quality care for older people with dementia or other mental health difficulties.

Post‐diagnostic follow‐up, signposting and education for patients and carers.

Support of the wider community team, including nurses, doctors, and psychologists.

Answer incoming calls, refer and signpost to agencies as required.

Review mental state, identify risks and safeguarding concerns.

Ensure accurate and timely recording of service user information.

Co‐develop care plans with patients and carers to achieve agreed goals.

Take baseline physical observations.

Qualifications and skills:

Literacy and numeracy skills.

Previous experience in care.

Basic computer skills.

Full UK driving licence B and access to a car for full‐time use.

Right to work in the UK.
