Complex care Support worker

Job OverviewWe have an exciting opportunity for dedicated and experienced Support Workers to join our compassionate team at Yorkshire Community Care, supporting our client in the Leeds & Bradford area.

Duties

Deliver personalised, high‐quality care and support tailored to the client's individual needs.

Assist with daily living activities including personal hygiene, mobility and meal preparation.

Administer medication and monitor health and wellbeing, responding appropriately to any changes.

Support community access, accompanying the client on outings, ensuring accessibility and safety.

Provide emotional support, encouragement and companionship.

Maintain accurate records of all care provided and report any concerns promptly.

Work collaboratively with family members and healthcare professionals to ensure a holistic approach to care.

What We're Looking For

6 months' experience in complex care.

Excellent communication and interpersonal skills.

Experience in tracheostomy and ventilation.

Peg feed & Medication administration.

Energetic, positive, and empathetic personality.

Awareness of accessibility needs, including wheelchair‐friendly routes, transport and venues.

Ability to work both independently and as part of a wider care team.

Shift Pattern: Mon–Sun, 12 hours a day, with at least one alternate weekend.

Plus an additional sleep shift.

A calm, resilient and proactive approach to complex situations.

Someone who can bring warmth, humour and creativity to the role.

NVQ Level 2 in Health & Social Care (desirable but not essential).
