We are looking to recruit an enthusiastic Health Care Assistant who is genuinely interested in providing safe and effective care for patients at home. The successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change. Flexible working will be considered for all roles.
Main duties of the jobThe role requires the staff member to work as part of a Community Nursing Service to deliver high‐quality care to patients in their own homes in accordance with individual care plans. Provide personal and palliative care, ensuring patients are treated with dignity, respect and compassion while supporting their comfort and wellbeing. Demonstrate the ability to work independently and autonomously within the community setting, effectively manage responsibilities while maintaining a high standard of care. Support the Community Nursing Team by managing the ordering and organisation of clinical supplies and stationery to ensure services operate smoothly. Consistently follow Trust policies, procedures and professional standards in all aspects of work while maintaining strict adherence to health and safety regulations, infection prevention and control guidelines. Ensure patient confidentiality is always maintained and information is handled sensitively and responsibly in line with legal, ethical and organisational requirements. Contribute to a safe, supportive and professional care environment for patients receiving community‐based nursing services.
Job responsibilitiesJob Purpose: To assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting.
Duties and Responsibilities:
Work as part of an integrated Community Nursing Service in providing specific care to a patient in the community setting as written in the care plan.
Provide personal care and support to palliative patients in their own homes.
Work alone and unsupervised, demonstrating the ability to work autonomously.
Be responsible for the ordering of clinical supplies and stationery for the Community Nursing Team.
Work in accordance with Trust Policies and Procedures.
Maintain high standards of personal hygiene in line with Health and Safety Guidelines and Infection Control Guidelines within the Trust.
Respect confidential information during the course of your work and refrain from discussing such information without the consent of the patient or a person entitled to act on their behalf, except where disclosure is required by law or court order.
Clinical
Monitor skin integrity of patients at risk of pressure ulcers and provide basic preventative information.
Provide full personal care to patients in the terminal phase of life.
Conduct health screening for household patients – e.g., monitoring blood glucose, phlebotomy, blood pressure recording and urine testing.
Provide clinical intervention for care of patients as directed by Registered Nurses, following training and assessment of competencies (e.g., administration of insulin, basic wound care and bladder irrigation).
Check equipment on a regular basis to ensure it is safe to use.
Attend mandatory training in accordance with the Trust Policy.
Identify areas for development within a Personal Development Plan in collaboration with the District Nurse coordinator/District Nursing Sister.
Report any Health and Safety or Infection Control issues.
Assist in clinical practice as expected up to NVQ Level III or equivalent.

The postholder must comply with Trust policies that protect the personal data of service users and employees from unauthorised disclosure, modification, destruction, inappropriate access or use and must pay due regard to the requirements of the NHS Confidentiality Code of Practice.
The postholder will be responsible for maintaining the clinical and/or corporate records that fall within the remit of this role to the standards set out in the Trust records management policies, and data quality processes and standards.
Postholders with elevated access to IT systems are held accountable to the highest standards of use.
Person SpecificationQualifications
GCE/GCSEs Grade A*‐C / Level 9‐4 in Maths and English or equivalent, or willingness to undertake Level 2 Functional Skills qualifications in Maths and English.
Care certificate.
Skills and Knowledge
Excellent communication skills, verbal and written.
Wound care and pressure ulcer knowledge/experience.
Ability to provide personal care to patients in the terminal phase of life.
Ability to undertake clinical intervention for care of patients as directed by Registered Nurses.
Ability to work well within a team.
Evidence of time‐management skills and ability to prioritise.
Use of SystmOne/EMIS.
Experience
Demonstrable experience of working in a healthcare setting.
Community healthcare experience.
Relationships

Job Specific Requirements
The ability to travel independently across the Trust.
The ability to work flexibly in accordance with service needs.
