OverviewLarch C is a 28-bed elderly care ward. Patient safety is embedded in everything we do. Our aim is to provide an excellent patient experience from admission to discharge, ensuring all physical and psychological needs are met.

ResponsibilitiesShift cover including night duty, weekends and bank holidays.Assist with personal care, continence and hygiene, promoting independence.Perform delegated clinical observations, supportive interventions and clinical skills directed by registered professionals.Maintain accurate records in electronic or paper systems and report changes or concerns to the appropriate professional.Participate in multidisciplinary team (MDT) meetings and liaise with patients, families, carers, and other professionals to ensure integrated quality care.Advocate for patients, respecting their wishes, dignity, and beliefs.Support rehabilitation and care planning, including end‐of‐life and discharge planning.

Person SpecificationQualificationsLevel 3 vocational qualification or equivalent in a range of patient and clinical care duties.Complication workbook equivalent to Level 3.National Care Certificate or commitment to complete within 12 weeks of commencing employment.Literacy and numeracy Level 2.Relevant additional training for Clinical Support Workers.

Knowledge & ExperienceExperience in health or social care.Experience in clinical observations and escalation of findings.Working knowledge of legislation underpinning care, including data protection, health & safety, and safeguarding.Understanding of documentation principles and GDPR confidentiality.Familiarity with NHS and social care policies, clinical policies and procedures, and Trust business.Experience working within the Trust.

Skills and AptitudesExcellent interpersonal, communication, and listening skills.Ability to adapt communication style.Time‐keeping, planning and prioritisation skills.Basic IT skills for electronic patient records and trust systems.Analytical and judgement skills for patient / service user interactions.Demonstrate holistic person‐centred care and initiative.Compassionate, empathetic, and dignified care.Motivated with a proactive approach, independent and as part of a team.Ability to work calmly under pressure.Professional image and courtesy.Venepuncture skills.

Personal CircumstancesWilling to work flexible hours including nights, weekends and bank holidays.Flexible and versatile approach to meet the service needs.Able to travel between Trust sites and attend off‐site meetings or training.

Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a Disclosure and Barring Service (DBS) check. The cost of the DBS will be deducted from salary through four‐month instalments, or as a one‐off payment. Standard and Enhanced checks are available with respective fees of £28.00 and £56.00. DBS charges will be deducted from salary over the following four months or as a one‐off payment.
