Overview Senior Support Worker to work in Older Adults Community Mental Health Teams at Hertfordshire Partnership NHS Trust. The role supports the Integrated Care Team, works alongside Discharge & Assessment nurses, and delivers physical and mental health care to older adults in a community setting.Responsibilities Champion the use of new digital technology (Whzan Blue Box) to maximise remote and home physical health monitoring.Facilitate annual physical health checks required for cardio‐metabolic screening of service users living with Serious Mental Illness and prescribed high‐risk psychotropic medications.Work as part of the Multi‐Disciplinary Team to assess the physical health needs of a diverse population of service users over 65, within their home environment.Undertake physical health checks and wellbeing assessments for people with severe mental illness.Promote and encourage healthy lifestyles, assisting service users to access support for recovery from mental and physical health problems.Support the locality in ensuring the Severe Mental Illness (SMI) registers are updated.
