NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non‐clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.
The RoleThe Long‐term Conditions service is an AHP‐led, digital‐first self‐management service. Patients referred to the team have been diagnosed with a long‐term health condition which has impacted their quality of life and ability to work. Common conditions could include ME/CFS, fibromyalgia, post‐viral fatigue and Long Covid, etc.
Care is provided via a 6‐week virtual self‐management group, specialised workshops and targeted 1‐1 interventions (often via near‐me). This is a hybrid role; therefore, work will be undertaken remotely from home, through community home visits, and by attending one of our remote working bases (Rowanpark or Belmont) or local health centres, as required. The Long‐term Health Conditions Service works across 6 HSCPs within NHSGGC boundaries. A driving licence and access to transport to support visits across this geography are needed.

All staff within the Services must comply with NHSGGC Policies. In addition to this, qualified staff will comply with the Chartered Society of Physiotherapy (CSP) or Royal College of Occupational Therapists (RCOT) Professional Standards and Code of Conduct and HCPC Standards of Proficiency. Health Care Support Workers are required to comply with the NHSGGC Mandatory Induction Standards and Code of Conduct.
Duration, Location, and Working PatternFixed Term until April 2026–March 2027 – Full‐time. Base location – Hybrid/ touch‐down space at Rowan Business Park.
Key ResponsibilitiesDirect Patient Care & Rehabilitation
Works with patients individually (virtually, in their home or in an agreed setting), delivering delegated supported self‐management treatment and rehabilitation programmes.
Adjusts treatment plans within agreed parameters and reports any unexpected changes in progress.
Supports patients with a wide range of needs, including complex fatigue, reduced quality of life, pain, breathlessness, etc.
Will support the provision and organisation of the virtual 6‐week self‐management group.
Will work with an individual to develop a wellness recovery action plan (WRAP) to support self‐management on discharge from the service.
May perform manual handling and use mobility/therapy equipment safely and competently.
Implements therapeutic activities such as memory strategies, relaxation, anxiety management, fatigue management, and other self‐management techniques.
Clinical Monitoring & Interventions
Carries out delegated clinical tasks.
Uses interdisciplinary assessment tools.
Coordinates interdisciplinary interventions from assessment through to discharge under supervision.
Provides health promotion information within a set framework.
Communication & Team Collaboration
Liaises with multidisciplinary team members and attends team meetings to provide updates on patient progress.
Completes referrals to other services and maintains communication with external agencies.
Provides advice and training to patients and carers on lifestyle changes, environmental adaptations, and the impact of disability.
Documentation, Data & Governance
Maintains accurate patient records, documentation, and statistical data (including electronic systems).
Collates and presents outcome‐measure data and supports audits, quality improvement, and clinical governance activities.
Ensures confidentiality in line with Data Protection legislation and organisational policies.
Equipment, Stock & Administrative Support
Manages stock levels, orders supplies, and completes associated administrative tasks.
Uses clinical equipment requiring dexterity and safe handling.
Training, Supervision & Service Development
Participates in supervision, eKSF, and personal development planning.
Supports the induction and training of new staff, students, and healthcare support workers.
Contributes to teaching sessions, in‐service training, and structured information sessions for clients.
Mentors students on placement and helps develop frameworks for monitoring care standards.
Safety & Professional Standards
Adheres to Health & Safety policies to maintain a safe environment for patients and staff.
Actively participates in service development and supports the efficient running of the Long‐Term Conditions Service.
Knowledge, Training, Qualifications and/or Experience Required to Do the JobEssential Criteria
Experience working within a Health and/or Social Care setting.
Full, current driving licence.
Effective verbal and written communication skills.
Working knowledge of basic IT systems.
Desirable Criteria
SVQ Level 3 (or equivalent) or a higher qualification relevant to clinical work.
Experience working with adults in a multidisciplinary, community‐based health/rehab service.
Ability to work independently.

From 1 April 2026, the Agenda for Change full‐time working week will reduce from 37 hours to 36 hours. Part‐time working hours will be reduced on a pro‐rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
All roles are subject to the NHS Agenda for Change and NHS pay scales. Work‐life balance is respected and flexible working options are available where service needs allow.
