Support Worker in Complex CareMidlands Partnership NHS Foundation Trust

In the Community Complex Care Team we provide patient and family‐centred care in a variety of settings. The role requires working within the Stoke‐on‐Trent community, covering a 24‐hour period, and includes days, nights and unsocial working times. You will work with one client throughout your shift, providing high‐quality, complex care.

Full training will be given to meet the individual's needs, but previous care experience is required.

Main duties of the job

Deliver high‐quality care in patients' homes or designated settings, including management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration.

Assess patient care needs daily, implementing and evaluating care plans produced by qualified nurses with patients and their families.

Review care plans, risk assessments and contingencies regularly in conjunction with qualified nurses, ensuring consistency and up‐to‐date care needs.

Ensure delivery and quality of care complies with local and Trust‐agreed policies and procedures.

Assist patients in achieving their potential to live independently as per the care plan.

Qualifications

NVQ3 or Nursery Nurse qualification or equivalent

NVQ4 / Diploma in a related subject

Experience

Significant experience in a similar role

Experience with complex care needs (tracheostomies, ventilators, enteral feeding, complex seizures and medication administration)

Demonstrated management of tracheostomies or ventilators

Additional informationSalary: £28,392 to £31,157 per annum, plus additional payment for unsocial working.
