Main duties of the job
To undertake non-complex clinical procedures under the guidance of senior team members e.g. Venepuncture, patient specific sub-cutaneous injections, sample collection, blood pressure monitoring, medications, wound care, Catheter care and other tasks according to training and competencies. To recognise their responsibility and accountability in accepting the delegated task.
Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team.
Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided.
To keep accurate, timely records according to Trust policy.
To escalat[e] and report patient progress/ status on a shift by shift basis to senior team members
To demonstrate flexibility and adaptability working across community teams according to service needs.
To commit to undertaking training and education to progress clinical skills as indicated by service development needs
To respond effectively and actively to complaints and elevate appropriately following departmental policies.
To assist registered staff in care and treatment and participate in the review process.
To promote equality for all, including the maintenance of confidentiality, supporting individual customs, values and spiritual beliefs and ensuring that anti discriminatory practices are demonstrated.
Job responsibilitiesYou would have training also in wound management and Leg Ulcer Management and compression bandaging. All HCAs are skilled up in observations, not just Blood Pressure, Temperature, respirations and Blood Glucose monitoring but taking a more holistic approach when attending to any patient and using the Risk Assessments required for all patient care.
Be able to take instruction from senior clinicians and be able to update on any patient you may have visited for treatment each day.
Person SpecificationGeneral Knowledge and Skills
Good Standard of Education
NVQ level III in Health Care or equivalent experience
Care Certificate completed or willing to work towards
Basic numeracy and ability to write simple reports
Health and Social Care Experience
Current experience of working in a caring environment, either in health, social or voluntary services
Experience of working with adults and/or experience of working in community or home care services.
Basic understanding of statutory health and social services.

£25,760 to £27,476 a yearPer annum Pro-rata.
