Job SummaryAn exciting opportunity for two part‐time Band 2 Support Workers to join the One Coventry Integrated Team, a community‐based multi‐disciplinary team that delivers reablement support to patients presenting with an acute deterioration in health and/or functional ability.

Responsibilities

Provide reablement support to patients, aiming to maximise independence and self‐management.

Identify when further support is required and signpost patients to appropriate long‐term services.

Collaborate with a wide range of health and social care professionals – including Advanced Clinical Practitioners, Clinical Practitioners, Nurses, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers, and other support workers.

Participate in induction, appraisal and ongoing training, and receive clinical and managerial supervision.

Requirements

Band 2 Support Worker qualifications and experience.

Full, valid UK driving licence with access to a vehicle for business use.

Current immunisation and vaccination report from a UK occupational health provider or GP.

Working HoursPart‐time – up to 30 hours per week across 5 days out of 7. Shifts available: Early 08:00‐14:00 and Late 16:00‐22:00.
