The RoleWe are looking for a compassionate, organised and proactive Pathway Coordinator to join our dedicated team in Seaforth, Liverpool.
This is an opportunity to make a genuine difference every single day by supporting people on their journey towards independent living and creating clear pathways into long-term accommodation and stability.
You will deliver structured, person-centred support to individuals within supported housing, helping them progress through tailored support plans, move-on pathways and practical life skills development.
Key Responsibilities- Deliver person-centred support and progression plans
- Support individuals towards independent living and move-on accommodation
- Carry out support planning, reviews and risk assessments
- Support access to training, employment and life skills opportunities
- Work collaboratively with external agencies and partners
- Maintain accurate case records and support documentation
- Promote safeguarding, wellbeing and positive community engagement
- Support housing management responsibilities where required

- Homelessness services
- Mental health or substance misuse support
- Safeguarding and risk management
- Person-centred support planning
