About the Role
The postholder will provide programme support to the healthcare public‐health portfolio within the Live Well, Age Well team, working closely with the Public Health Strategist to deliver projects and priorities focused on equitable access, culturally responsive delivery and improving outcomes for underserved communities.
Supporting the delivery of the CVD Prevention Programme
Contributing to NHS Health Checks and equitable access
Supporting work on Diabetes Prevention inequalities projects
Delivery of Making Every Contact Count (MECC) programme
Supporting work on Dementia and Age Well programmes
About You
• Strong commitment to tackling health inequalities and promoting culturally responsive delivery
• Experience in project management to monitor progress, engage stakeholders and ensure quality outcomes for multi‐agency public health projects
• Excellent interpersonal and strategic partnership skills, with the ability to work effectively across organisational boundaries
• Strong understanding of population health, disease prevention, integrated care systems, and utilisation of effective healthcare interventions
• Adaptable and resilient in a dynamic environment with an outcome‐driven focus
