We have an exciting opportunity for a Community Specialist Practitioner student to work in District Nursing within Trafford Local Care Organisation.
This is a 53‐week Level 7 post‐graduate diploma (District Nursing). Fixed‐term 53‐week contract. Paid at Band 5 for the duration of the course.
Our teams work as one in the neighbourhoods they support, delivering and designing services in partnership with local people. We also provide key services that work across wider localities and city‐wide. The Community Specialist Practitioners' course prepares practitioners for their role as a District Nurse, meeting professional standards outlined in NMC (2023) Standards of Proficiency and QNI/QNIS voluntary standards (2016).
This opportunity is in conjunction with Greater Manchester University (formerly Bolton University).
You will have a current first‐level registration on part one of the NMC Register and at least 120 credit points at level 6 or APEL equivalent.
Main duties of the job Manage an integrated caseload of patients within the neighbourhood team, providing support and care to patients with varying complexity and intensity of needs, promoting health and wellbeing, self‐care and independence.
Promote clinical excellence and evidence‐based approach to care by complying with agreed Trust policies, maintaining own clinical competencies and developing those of others in a clinical leadership role.
Develop skills to support management of the clinical area alongside sisters within the team.
Monitor agreed induction, orientation and preceptorship programmes for newly appointed staff and students.
Attend University of Manchester and complete a clinical placement in a designated area, supported by a Clinical Supervisor and Assessor. Ratio of university attendance to clinical practice is approximately 60:40.
Qualifications Current first‐level registration on Part 1 of the NMC Register.
At least 120 academic credits at level 6 or equivalent experience.
Evidence of ability to study successfully at level 6 (APL considered).
Up‐to‐date professional portfolio demonstrating reflective, evidence‐based practice and continuous professional development.
Relevant training to degree level or above (e.g., V150, research, palliative care, elderly care, enhanced communication skills, teaching, leadership and management).
Experience 1 year community experience at Band 5 or above.
2 years post‐registration experience.
Skills Excellent communication skills in patient care and team working (verbal, non‐verbal and written).
Organised professional portfolio with reflective practice and service‐user feedback.
Aptitude to work cohesively within a team.
Computer literacy skills relevant to education and practice.
Sound knowledge of health policy relating to chosen pathway.

Salary £32,073 to £39,043 a year (pro‐rata).
