Anexciting opportunity has arisen to join our Community Care Hall Green PCN team.We are a dynamic, and forward thinking PCN, who are passionate about developingand delivering excellent quality local services to meet the needs of ourpatients.

Weare looking to expand our PCN team by seeking to employ a General Practitioner to work alongside our practices and PCN staff. As well as general practice staff, ourPCN staff group currently also includes Clinical Pharmacists, PhysiciansAssociates, Social Prescribers, Physiotherapists, Mental Health Practitioners, Care Coordinators, Nurse Associates and Pharmacist Technicians.

Main duties of the job

Toperform as an autonomous general practitioner working independently and withother health care professionals to assess, diagnose and treat the conditions ofpatients attending within a primary care setting.

Provideexpert professional advice to patients, carers and colleagues and ensure themaintenance of clinical excellence.

Tobe professionally accountable and responsible for all elements of clinicalpractice in accordance with the GMC code of conduct, ensuring the safety ofpatients and the quality of their care is the main focus.

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients across a Primary Care Network of 3 GP Practices, dealing with a diverse range of health needs. The post holder will be based at Northbrook Group Practice.

This will include, but is not limited to, surgery and telephone consultations, on call, home visits, administration, complying with the practices contracts (GMS/PMS/enhanced services/DES).

Job responsibilities

ClinicalResponsibilities are as below

Inaccordance with the Practice timetable, as agreed, the post-holder will beavailable to undertake a variety of duties including surgery consultations,telephone consultations, visiting patients at home, checking and signing repeatprescriptions and dealing with queries, paperwork, reports and correspondencein a timely fashion

Triagepatients presenting to the surgery by telephone, online and in person when oncall

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation

Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems

Screeningpatients for disease risk factors and early signs of illness

Inconsultation with patients and in line with current Practice disease managementprotocols, developing care plans for health and promoting healthy lifestylestrategies and education

Managelong-term conditions and patients with complex problems

Recordingclear and contemporaneous consultation notes to agreed standards

Collectingdata for audit purposes and complying with QOF requirements

Check,manage & process patient test results

Referpatients to secondary care and/or relevant organisations and internally withinthe PCN (e.g. To additional roles members of the PCN team) as required

Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible) in line with local and nationalguidelines

Prescribingin accordance with the Practice prescribing formulary (or generically) wheneverthis is clinically appropriate

In general,the post-holder will be expected to undertake the duties and responsibilitiesassociated with a GP working within primary care in a surgery on a PCN basis

  • Understanding current NHS policies

  • Knowledge of the demographic characteristics that affect health and health care in the area including health inequalities

Qualifications

  • MBBS or equivalent medical degree
  • Full registration with GMC
  • Certification of Completion of Training (JCPTGP/PMETB)
  • Inclusion on Performers List
  • Evidence of a commitment to on-going personal & professional development

Other

  • Committed to working as part of a multidisciplinary, multi-agency primary care team
  • Openness to explore alternative working practices
