Job description

We are offering an exciting opportunity for 5 sessions per week, Tuesday, Thursday and Friday. You will be supported by our friendly team of clinicians; 6 GP partners, 8 salaried GPs and a large nursing team. Our clinical team work closely to support each other, and regular team meetings are supported by the practice where our clinicians can communicate and suggest new and improved ways of working.

Main duties of the job

Services to be provided by you:

Face to face appointments, telephone triage of patients, covering duty days, home visits when required, telephone appointments, complete all necessary documentation relating to patient care, including referral letters, medical reports, and certification of sickness.

Job responsibilities

To provide NHS general medical services toregistered and temporary patients on the Practice list.

In consultation with patients and in line withcurrent Practice disease management protocols, developing care plans forhealth.

To assist in the establishment of appropriatesystems to manage common chronic medical conditions.

Completion of clinically relatedadministrative and non-clinical duties needed for the delivery of service.

Refer patients for further opinions and interventions according toPractice protocols.

CLINICALRESPONSIBILITIES:

Undertake a variety of duties, including surgery consultations,telephone consultations and queries, visiting patients at home, checking andsigning repeat prescriptions and dealing with queries, paperwork andcorrespondence.

Making professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation.

Assessing the health care needs of patients with undifferentiated andundiagnosed problems.

Screening patients for disease risk factors and early signs of illness.

Developing care plans for health in consultation with patients and inline with current practice disease management protocols.

Providing counselling and health education.

Admitting or discharging patients to and from the caseload and referringto other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreedstandards.

Compiling and issuing computer-generated acute and repeat prescriptions.

Prescribing in accordance with the practice prescribing formulary (orgenerically) whenever this is clinically appropriate.

In general, the post-holder will be expected to undertake all the normalduties and responsibilities associated with a GP working within primary care.

Maintaining,monitoring, and updating computer database and management of medical recordsand practice audits:

Ensure that practice clinical database is updated promptly andaccurately with information relating to patient records, as required for clinicalaccuracy and for practice recording requirements under QOF and EnhancedServices.

Appropriate use of read codes.

Participating in the collection and collation of statistics towards thepreparation of practice reports, performance monitoring and audits.

Participating in the auditing of practice activity.

Practiceorganisation:

Process and action incoming patient and hospital correspondence

Attendance at in-house meetings, significant event audit meetings andothers as deemed necessary

Ensure that the practice complaints system is adhered to at all times

Awareness and compliance with all relevant guidelines e.g. Dataprotection, confidentiality and health and safety
