Hebden Bridge Group Practice is a large, forward-thinking and well-established GP practice serving just under 19,000 patients across three sites in the Upper Calder Valley.

Job responsibilities

JobSummary:

The post-holder will manage a caseload anddeal with a wide range of health needs in a primary care setting, ensuring thehighest standards of care for all registered and temporary patients.

ClinicalResponsibilities:

  • In accordance with the Job Plan as agreed, the post-holder willmake him/her-self available to undertake a variety of duties includingsurgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions and dealingwith queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcare workers within the organisation
  • Assessing the health care needs of patients withundifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs ofillness
  • In consultation with patients and in line with current Practicedisease management protocols, developing care plans for health
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload andreferring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes toagreed standards using the Practice electronic clinical system
  • Compiling and issuing computer-generated acute and repeatprescriptions
  • Prescribing in accordance with the Practice prescribingformulary (or generically) whenever this is clinically appropriate
  • In general the post-holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working withinprimary care.

OtherResponsibilities within the Practice:

  • Awareness of and compliance with all relevant Practicepolicies/guidelines, eg prescribing, confidentiality, data protection,health and safety
  • A commitment to life-long learning and audit to ensureevidence-based best practice
  • Contributing to evaluation/audit and clinical standard settingwithin the Practice
  • Contributing to the development of computer-based patientrecords
  • Contributing to the summarising of patient records and Read-Codingpatient data
  • Contributing to the Practice achievement of QOF and otherperformance targets
  • Attending training and events organised by the Practice orother agencies, where appropriate.

Person Specification

Qualifications

  • GMC registered
  • MRCGP

Experience

  • GP training and experience
  • Evidence of learning from significant events analyses
  • Willing to develop an interest in a specific disease area
  • Use of a GP clinical system
  • Use of SystmOne

Knowledge and Skills

  • Understanding of the Quality and Outcomes Framework (QOF)
  • Interest in GP training and education
