We are looking for a compassionate, reliable, and motivated Outreach Support Worker to join our dedicated team. In this rewarding role, you will support adults withlow-level mental health needs and orlearning difficulties, helping them to live as independently as possible within their local community.

Key Responsibilities- Provide high-quality, person-centred support tailored to individual needs.
- Support individuals to develop and maintain independent living skills.
- Assist with accessing the community, attending appointments, and participating in social activities.
- Promote choice, dignity, independence, and inclusion at all times.
- Support individuals with emotional wellbeing and low-level mental health needs.
- Maintain accurate records and complete daily documentation.
- Work collaboratively with colleagues, families, and other professionals to achieve positive outcomes.
- Follow safeguarding procedures and organisational policies.
- A minimum of 12 months' experience in a similar support worker or outreach role.
- Experience supporting adults with learning difficulties and/or mental health needs.
- A full UK driving licence and access to a vehicle is essential, as this role involves travelling between service users in the community.
- Excellent communication and interpersonal skills.
- A caring, patient, and positive approach.
- The right to work in the UK.
