Were recruiting Support Workers to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.This role supports people returning home from hospital to recover safely and regain independence doing with, not doing for.LocationHerefordshire (community-based)What youll be doingSupporting people at home after discharge with reablement-focused careAssisting with personal care where needed, while encouraging independenceSupporting daily living skills (washing, dressing, meal prep, mobility, routines)Following care plans and recording notes accuratelyReporting changes in condition, concerns, or risks promptlyWorking respectfully with families and professionals as part of a wider teamWhat were looking forCaring, reliable and professional approachExperience in home care is helpful, but not essential (full training provided)Good communication and respect for dignity, choice and independenceFull UK driving licence and access to a vehicle (essential for Herefordshire)Right to work in the UKWhat we offerCompetitive pay (depending on experience)Paid training and ongoing development (including reablement best practice)Supportive management and a friendly teamProgression opportunities (Senior Support Worker/ Team Leader)TPBN1_UKTJ
