Three Counties Care Services Ltd is looking for full time compassionate, reliable and dedicated Support Workersto join our team in Cambridge.

This is an excellent opportunity for someone who is passionate about supporting people to live safely, confidently and as independently as possible. As a Support Worker, you will provide person-centred support tailored to each individual's needs, choices and goals. You will play an important role in promoting independence, dignity, wellbeing and inclusion while helping the people we support get the most from everyday life.

The Role

Your responsibilities will include:

  • Providing high-quality, person-centred support in accordance with individual care and support plans
  • Supporting individuals with everyday living skills and encouraging independence
  • Assisting with shopping, meal preparation, household tasks and appointments
  • Supporting individuals to access the community, activities, education, employment and social opportunities
  • Providing emotional support, encouragement and companionship
  • Supporting individuals with personal care where required
  • Supporting with medication in accordance with care plans, training and company procedures
  • Promoting choice, dignity, privacy and independence
  • Building positive and professional relationships with the people we support
  • Recognising and responding appropriately to safeguarding concerns
  • Monitoring wellbeing and reporting any changes or concerns
  • Maintaining accurate daily records, reports and other documentation
  • Working effectively with colleagues, families and external professionals
  • Following risk assessments, support plans, policies and agreed procedures
  • Working flexibly as part of the staff rota, which may include evenings, weekends and sleep-in shifts

Previous experience withinsupported living, residential care, learning disabilities, autism, mental health or a similar health and social care settingwould be advantageous. The role involves working as part of a team and undertaking sleep-ins.

A Level 3 qualification in Health & Social Careis required, although applicants with the right values, attitude and relevant experience are also encouraged to apply.
