Clinical Pharmacist – Full Time (Job Share Considered)

£24.45 – £28.48 per hour

Hybrid working available - 3 remote/2 on site

Healthii People are recruiting on behalf of a forward-thinking GP practice who are looking for a Clinical Pharmacist to join their pharmacy team.

What you’ll do:

• Work as part of a multi-disciplinary team in a patient-facing role.
• Lead on areas of chronic disease management and conduct structured medication reviews.
• Provide clinical medicines expertise to GPs and practice staff, supporting safe prescribing and medicines optimisation.
• Support patients with complex polypharmacy, ensuring they get the best outcomes from their medication.
• Liaise with care homes, community pharmacies, and secondary care to improve medicines safety and continuity of care.
• Undertake audits, review prescribing data, and contribute to the development of new services.
• Provide training, education, and clinical support to colleagues.

About you:

• Qualified Clinical Pharmacist with primary care experience (or working towards it).
• Strong communication skills and ability to work across a multi-disciplinary team.
• Passionate about patient care, medicines optimisation, and continuous improvement.
• Organised, proactive, and able to balance autonomy with teamwork.
