Job description

Service Care Solutions are seeking an experienced Advanced Clinical Practitioner (ACP) to join an innovative and established Frailty Team. You’ll work autonomously at an advanced clinical level, delivering person-centred care to frail and vulnerable patients in their homes and communities, supported by a multi-disciplinary team.

Location: Scarborough and surrounding areas
Contract Type: Fixed Term – 24 Months
Salary: Competitive (Per Session, based on experience)
Hours: Full-time / Part-time considered
Reports to: Director of Community Services

Based in our Scarborough, you will provide leadership in delivering holistic care that helps reduce hospital admissions and facilitates early discharges for complex patients.

Key Responsibilities:
  • Independently assess, diagnose, treat, and manage patients with complex needs

  • Prescribe, refer, admit and discharge within scope of practice

  • Lead service development, quality improvement, and audit initiatives

  • Collaborate across sectors and services to ensure joined-up patient care

  • Engage with strategic frailty initiatives across the system


Required:
  • Registered healthcare professional 

  • MSc in Advanced Clinical Practice (or equivalent portfolio)

  • Independent Prescriber qualification

  • Experience managing frailty, complex care or urgent community services

  • Confident leader and educator with strong decision-making skills

  • Knowledge of current frailty frameworks, national policy and integrated models of care
