Community Healthcare Assistant - Carer

Job description

Summary of RoleTo provide high-quality, person-centred care and support to individuals in their own homes, promoting independence, dignity, choice and wellbeing at all times. The post holder will be expected to deliver care in line with individual care plans, company policies, and regulatory standards.

Key Responsibilities

Provide assistance with personal care, including washing, dressing, grooming and continence care

Support individuals with mobility and safe transfers, using appropriate equipment where trained

Assist with medication administration in line with training and policy

Promote independence and encourage service users to do as much as they are able

Support with meal preparation and nutrition

Assist with household tasks including cleaning, laundry and shopping

Support individuals to attend appointments and engage in community activities

Provide companionship and emotional support

Support individuals to develop life skills and maintain independence

Work collaboratively with on-site staff and multidisciplinary teams

Follow structured care routines within supported or shared environments

Maintain professional boundaries at all times

Monitor and report any changes in health, behaviour or wellbeing

Follow care plans, risk assessments and safeguarding procedures

Escalate concerns promptly in line with company policy

Maintain infection prevention and control standards

Complete accurate and timely care records

Communicate effectively with service users, families and professionals

Participate in handovers and team meetings as required

Work in accordance with CQC regulations and company policies

Maintain confidentiality and data protection standards

Attend mandatory training and supervision

Uphold equality, diversity and inclusion principles

Represent the organisation professionally at all times

Person SpecificationEssential

Compassionate, reliable and professional

Strong communication skills

Ability to work independently or as part of a team

Willingness to undertake mandatory training

Right to work in the UK

Enhanced DBS (or willingness to obtain)

Desirable

UK Care Experience

NVQ Level 2 or 3 in Health & Social Care

Skills and Knowledge

Strong verbal, written communication, and interpersonal skills.

Excellent teamwork ability.

Great organisational skills and ability to prioritise workload.

Strong customer service skills.

Able to work in a team but use own initiative.

Able to follow instructions and procedures.

Willing to be hands on with patients.

Practical and able to prioritise workload.

Confident and possess a desire to learn.
