Job description
Key Responsibilities
- Case Management: Lead on individual learner cases, coordinating multi-agency support and ensuring timely interventions for those at risk of exclusion, disengagement.
- Pathway Planning: Develop personalised education and progression plans that reflect each learner’s strengths, interests, and aspirations
- Alternative Provision Oversight: Coordinate high-quality alternative education placements for students who cannot access mainstream settings, ensuring they are purposeful and lead to recognised outcomes.
- Multi-Agency Working: Attend and contribute to Team Around the Child (TAC) meetings, PEP reviews, and safeguarding discussions to ensure education remains central to planning.
- Attendance and Engagement Monitoring: Track attendance, progress, and wellbeing, intervening early to prevent drop-out or disengagement.
- Student Voice and Empowerment: Ensure young people are actively involved in decisions about their education and future, promoting confidence and self-advocacy.
Skills and Attributes
- Strong understanding of Children looked after and or Early years
- Knowledge of SEND, safeguarding, and statutory responsibilities
- Excellent communication, coordination, and case management skills
- Commitment to inclusive education and lifelong learning
