Clinical Team Lead - 22.5 hours part time/Job shareThe closing date is 18 May 2026.
We are looking for a Clinical Team Lead, an internal MPFT applicant only, to provide clinical leadership and advice to staff within the Community Learning Disability Team (CLDT). The role includes supporting nurses in complex case management, ensuring appropriate clinical and managerial supervision, managing community caseload and waiting list, and liaising with Clinical Nurse Specialists to deliver seamless services across the Specialist LD directorate.
Main Duties
Support and oversee operational delivery of the team.
Coordinate with the Business Support Manager to keep risk registers, contractual information and KPIs updated.
Work autonomously to address service delivery issues, with strategic direction from the Head of Service.
Provide clinical leadership and advice, acting as a visible point of call for staff.
Supervise members of the CLDT.
Ensure accountability regarding community caseload and waiting list.
Define the role and parameters around community nursing.
Support the team in complex case management.
Ensure MDT decision making aligns with clinical standards.
Manage duty and intake processes ensuring referral health coordination.
Address and progress performance issues with other professional leads.
Implement clinical processes aligned with the Clinical Strategy.
Lead on clinical audit and service improvement initiatives.
Work with the Head of Nursing and other leads to address highlighted issues.
Ensure quality of care for people with learning disabilities, identifying gaps when necessary.
Qualifications & Training
Qualified Nursing Practitioner RNLD or equivalent professional registration.
Degree or equivalent experience in a relevant subject.
Experience
Significant experience at Band 6 or above.
Experience working with statutory, voluntary or private sector organisations, including direct collaboration with local authority colleagues.
Experience in assessing clients with complex healthcare needs and evidence of staff development and appraisal.
Skills, Knowledge & Abilities
Application of care models and theories into practice.
Identify deficiencies in services and initiate development.
In-depth knowledge of the national agenda regarding the NHS, social care, learning disabilities and mental health.
Knowledge of mental health, physical health, positive behaviour support and applied behaviour analysis.
Skills required to deliver clinical supervision.
Knowledge of DOLS, mental capacity and the Mental Health Act legislation.
Personal Attributes
Practice ethically, considering the needs and best interests of the person receiving care.
Excellent communication awareness, skills and capability to present information in a clear and concise manner.
