Advanced Clinical Practitioner - Part-timeThe ACP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
They have been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The post-holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals setting and job role.
Main duties of the jobThe ACP will work as part of a multidisciplinary team to see, treat and refer patients with acute and long term conditions.
ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. An ACP is autonomous in making decisions based on assessment, diagnosis and the interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

We offer primary medical services and long term condition monitoring, plus access to pharmacists, first contact physiotherapists, mental health practitioners, and minor surgery.
The practice operates a partnership model of 3 GP partners, who are supported by an ANP salaried partner who leads the nursing team and is involved in shaping the practice and its services, and a dedicated practice manager to run the business and operations.
The clinical team is well supported by a team of experienced receptionists trained in care navigation, and administrative support staff.
We received a rating of 'Good' in our last CQC inspection, and was rated best overall practice in Kirklees in the 2024 Patient Survey.

Job responsibilitiesThe following are the core responsibilities of the ACP in delivering health services. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
a. Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice
b. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information
c. Act on professional judgement about seeking help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change
d. Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate
e. Triage and treat patients wishing to see a health care professional
f. Make any necessary referrals to other members of the primary health care team or other agencies
g. Develop, implement and embed health promotion and wellbeing programmes
h. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
i. Implement and evaluate individual specialised treatment plans for chronic disease patients
j. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
k. Provide advanced, specialist clinical care to patients as required in accordance with clinical based evidence, NICE and the NSF
l. Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change
m. Use expertise and decision-making skills to inform clinical reasoning
n. Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care
o. Exercise professional judgement to manage risk
p. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
q. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
r. Request and collect pathological specimens. Upon receipt, process and interpret pathology and other test results as required
s. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
t. Maintain chronic disease registers
u. Prioritise health issues and intervene appropriately
v. Support the team in dealing with clinical emergencies
w. Recognise, assess and refer patients presenting with mental health needs
x. Support in the provision of vaccination programmes for both adults and children
y. Be an extended and supplementary prescriber, adhering to extant guidance
z. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
aa. Contribute to practice targets (QOF, etc.), complying with relevant guidance
bb. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
cc. Visit patients in their own homes, assessing their health, diagnosing and treating conditions, and referring to secondary care or other health professionals as appropriate
dd. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
ee. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
ff. Deliver opportunistic health promotion where appropriate

Excellent interpersonal, influencing, negotiation and organisation skills with the ability to constructively challenge the view and practices of managers and clinicians
Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
Knowledge of national standards that inform general practice (NSF NICE guidelines)
Experience
Experience of practice within the four pillars

Qualifications
Qualified healthcare professional that is educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England
Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
Proof of relevant professional and educational qualifications, coupled with details of any training
Registration with their professional governing body
Qualified independent prescriber, if not understand the requirement for PGDs and associated policy
