Job OverviewTo provide day to day care for service users under the supervision of a Clinician which adheres to best practice, standards and guidelines, and contributes towards the service users care and treatment.

Work in partnership with service users, other members of the MDT staff as appropriate, to achieve the optimum health outcome for clients.

Be part of the MDT in providing a high and consistent level of holistic care, maintaining the dignity and equality of the individual. This includes direct care.

Main duties of the jobHealth Care Support Workers are part of the Mental Health Team based in Heddfan Wrexham. They support elderly and vulnerable people with mental health problems such as dementia and depression, enabling people to stay living as independently as possible, participate in activities in their local community and maintain good mental and physical wellbeing.

We are looking for well‐motivated, conscientious and friendly people who are willing to learn and play a key role in improving the lives of the patients we work with. Contact is with patients, relatives, other members of the public and professionals from partner organisations.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Desirable criteria

Training in Mental Health.

ExperienceEssential criteria

Significant experience of working with people with dementia.

Desirable criteria

Worked in Mental Health care setting.

Experience of working in the community.

SkillsEssential criteria

Evidence of working as part of MDT.

Evidence of good communication skills.

Evidence of working autonomously.

Desirable criteria

Ability to speak Welsh.

Evidence of IT skills.
