Community Support WorkerOur teams support patients to live independently and improve their quality of life in the place they like to call home.
As a Community Support Worker, you will work closely with a multi‐disciplinary team including physiotherapists and nurses to provide ongoing clinical activities such as rehabilitation, recovery, nursing tasks, and support with daily routines and personal care.
We are also interested in hearing from occupational therapy or physiotherapy students nearing qualification looking to build confidence and gain valuable experience within community healthcare.
Main duties of the job
Undertake a range of delegated clinical, therapy & nursing‐led activities to support the service users on the Integrated Network Team caseload.
Provide a safe, patient‐centred and effective clinical activities in the community setting.
Work within adult services to deliver a full range of healthcare activities.
Be expected to work flexibly with other teams or services, both within the locality and Sirona wide when the need arises at short notice.
Be able and willing to travel throughout the service delivery geographical area.

£25,760 to £27,476 a year pro rata, per annum.

Working in South Gloucestershire LocalityBase location will be either Warmley Business Park (BS30 8XP) or Woodlands Court (BS32 4LB).
