The RoleAs a Support Worker in our Mental Health Outreach Service, you can make your own powerful contribution to a diverse team who share the same values and vision. Joining a team where everyone feels comfortable showing up to work as their whole self, you'll work with colleagues and teams who are involved in the care of your assigned service users, to make sure the best possible service is delivered.
As a key worker for users of our Mental Health Outreach Service, you'll identify their needs and work with them to develop care plans focused on achieving life‐enhancing outcomes. It's all about promoting independence and emotional wellbeing. You will develop respectful, supportive and sensitive relationships with service users. And you'll assess, plan, implement, manage and evaluate activities within a person‐centred planning framework, which will help people develop valuable life skills. Ultimately, you'll help people improve their mental health and wellbeing, while securing more control over their own lives.
To make a difference from day one, you'll need some experience in a health and social care role. You have worked with people who have severe and enduring mental health problems, and with their family and carers. You demonstrate an understanding of key principles which underpin the way we work; social inclusion, independence, empathy and empowerment. As well as the ability to plan and deliver varied activities, you are comfortable producing written reports and statistical data which show the difference we can make.

QualificationsEssential experience working in health and social care, with people who have severe and enduring mental health problems, and their families and carers. Understanding of principles such as social inclusion, independence, empathy and empowerment. Ability to plan and deliver varied activities, produce written reports and statistical data. For Care Leavers, meeting the essential criteria for the role will result in an automatic invitation to interview.
