This is a development post. You will be supportedto achieve a Level 2 qualification in Health & Social Care via anapprenticeship route, which includes dedicated time at college for formal study
As part of this role, you will be required tocomplete the course & work through all required competencies within anagreed timeframe, in order to meet the requirements of a Band 3 position.Failure to successfully complete the qualification & competencies mayimpact upon your continued employment
You'llwork under the guidance of registered healthcare professionals to deliver highquality, person-centred care on the Intermediate Care pathway.
You will promote independence with personal cares &activities of daily living, mobility practice & encouraging participationin physical exercise programmes & therapy plans
Atthe time of advertising, this role does not meet the minimum requirements setby UK Visas and Immigration to sponsor candidates to work in the UK. For thisreason, we are unable to sponsor anyone for a skilled worker visa for this role.

Main duties of the job
As RehabilitationSupport Worker you will
workboth independently & collaboratively supporting individuals in their ownhomes & care homes
Workin close liaison with the MDT, excellent communication & time managementskills are essential
Assistin initial assessment & development/implementation of care plans
Participatein training, education, & clinical supervision to develop skills &knowledge & apply continuous reflection on own practice
Maintain accurate clinical records
Support safe use of equipment suchas hoists, walking aids & pressure cushions
Use of a vehicle for travelrelated to the duties of this role is essential
This role involves a shift pattern, includingweekends & bank holidays
Applicants must be aged 18 or over and have been aUK resident for at least three years (student visa excluded) to meet theapprenticeship eligibility for funding. Checks will be carried out as part ofthe recruitment process.

JOB SUMMARY
The post holderwill:
Underthe supervision of registered healthcare professionals/associate practitioner, implement delegatedtasks identified within the therapy care plan, to include the basic, routinetherapeutic, psychological and physical needs of service users.
As part of thehealthcare team, provide the highest standards of care and compassionateplanned care to people that maximises their recovery and rehabilitationpotential.
Support service users with personal care activities (bathing,toileting, dressing, support with meals, assist patients with their appearance).
Assist clinicians in supporting people to carry outeveryday tasks.
Support in the delivery of the service inperforming routine administration or housekeeping tasks.
Develop andmaintain meaningful therapeutic relationships with people. Adopt a polite,positive and empathetic approach, providing and receiving routine informationappropriately.
Ensure the care experienced is positive and service user safety ismaintained. Treat everyone, eg,service users, their relatives, visitors, colleaguesetc, with respect, dignity and courtesy and in accordancewiththe Trusts Values.
Follow and ensure routine care is provided inaccordance with well-established policies, procedures, protocols, guidelinesand established clinical care plans.
Undertake the physical aspects of care eg, movingand handling of service users. Use items of equipment in delivering care eg,hoists, wheelchairs etc, (as appropriate to the role), ensuring appropriateinstruction on their use has been received.
Berequired to concentrate for periods of time, depending upon the careintervention/care plan. That includes undertaking personal care, engaging withservice users, documentation and raising issues of concern appropriately.
Participate in a flexible rostering arrangementproviding 7-day care, (as appropriate to the role).
Be able to respond to verbal / physical aggressionor challenging behaviour (including risk to self); recognising and respondingto changes in presentation, including reporting to a clinician as required inorder to maintain own personal safety and accessing support for service users.
Assistin the support of individuals who exhibit challenging behaviour or present asignificant imminent risk to themselves or others. Recognise and respond tochanges in their presentation, including reporting the issue to aregistered health practitioner/associate practitioner as required. Respond appropriately to any emergencysituations.
Identify and manage risks, including assessmentof moving and handling, using local policies and procedures. Is aware of theenvironment in which they work and how to maintain safety for themselves andothers.
